How it works
For funeral directors who need every personal fact right before the death certificate is filed, and hate calling the family back. Paste your first call and arrangement conference notes. Tribute Death Certificate Checker goes through the funeral director's part of the U.S. Standard Certificate of Death, from legal name, date and place of birth, residence and marital status to the spouse's and parents' names, informant, education, Hispanic origin, race, usual occupation and industry, place of death and the method and place of disposition, and marks each one you have, unclear or missing. Nicknames instead of legal names, a city without a state and dates that don't match are caught. You get a short list of gentle questions to ask the informant, and the permits and next steps for the disposition, like the cremation authorization and medical examiner clearance for a cremation or the burial transit permit for a removal out of state. The Social Security number is only marked as given or missing, never repeated. State forms and deadlines vary.
Fields follow the U.S. Standard Certificate of Death. Your state's form, permits and deadlines may differ.
